Here's some information which might help you
prepare for coming to the hospital.
What to Bring With You
Your Social Security Number
Medical and Prescription Insurance Cards and preauthorization
if you have one
Picture ID
Physician orders, test results for lab, EKS, and
X rays not done here
A list of current medications,
vitamins, and herbal medicines including name, dose,
and how often taken. It’s
a good idea to bring them with you and send them home
with a friend or relative after you are admitted.
A copy of your Advance Directives or
Living Will (copies are available
in admitting if you would like one). Also
find forms, a brochure that can be downloaded and
questions and answers on the Washington State Medical
Association web site at http://www.wsma.org/patients/who_decide.html
Personal hygiene items or other items such as curling
irons, radios or hair dryers
Loose, comfortable clothes for your stay and going
home
Personal items such as books, puzzles
Don't
bring
Valuables, jewelry, or cash (excluding payment for
deductible or co-insurance); if you cannot avoid having
these items with you, please have a staff member put
them in our safe
Avoid bringing contacts lenses if possible
Humidifiers, dehumidifiers, electric blankets, heating
pads and portable heaters
If You Wear Hearing
Aids, Eyeglasses, or Dentures
If you have a case for these items,
bring it along. If not, the staff will be happy
to get a cup and label it for you to prevent loss.
